Join our new project as a high intensity user outreach and intervention worker

NHS South Sefton and NHS Southport and Formby clinical commissioning groups (CCGs) are working with Sefton CVS to deliver a project to support high-intensity users of emergency NHS services.

We are seeking an experienced, motivated individual, with experience of working with people with complex needs as well as working within a fast-paced environment, to join our exciting project. The successful candidate will work with a rolling cohort of patients who attend A&E and are admitted to hospital more than might be expected. We are looking for individuals whose ability to connect with patients is strong enough to be able to change the behaviours of people with even the most complex medical/social needs. The post holder will seek to appropriately reduce the number of emergency ambulance calls and unscheduled care interactions, emergency contacts and other care interactions across South Sefton and Southport and Formby CCG areas.

Reporting to the High Intensity User Co-ordinator, the Outreach and Intervention Worker will work with people to focus on positive behaviour change addressing those issues that are contributing to inappropriate care seeking behaviours.

This role is fixed term until 30th September 2021 in the first instance and is subject to an enhanced DBS check.
