Sefton Medicines Optimisation in Care Homes

Our team of pharmacists and pharmacy technicians work directly with GP practices, care homes, the local authority, patients and pharmacies to ensure safe and effective medicines optimisation across Sefton for care home residents.

Our medicines management pharmacists complete medication reviews on behalf of Sefton GPs for care home residents. Medication reviews are particularly important for older people due to:

  2. age related changes in their major organs, which can alter how medicines work within the body.
  3. often having multiple conditions needing more medicines they need to take which often leads to interactions between medicines.
  • Supporting homes to comply with CQC Regulations (Regulation 9 - person centred care and regulation 12 – safe care and treatment)

Our medicines management technicians can complete a medicines audit in the home. This audit can be used as evidence to prove compliance with CQC regulations.
