The CHART team focuses on the accommodation and support needs of people in Sefton who have a severe and enduring mental illness. The team works to obtain suitable accommodation, particularly for those whose discharge from hospital is being delayed.

Living Well Sefton

Living Well Sefton is a free service with a focus on supporting people with issues that may be affecting their health and wellbeing. It is collaboration of various Sefton organisations with the expertise and knowledge to benefit residents.

As part of the service there are a number of Living Well Mentors who work with residents on a one-to-one basis to identify health areas where small changes can be made but may have a big impact on improving health, such as exercise, cooking and eating better, stopping smoking, coping better with stressful situations, help with debt and benefits and more.

Sefton Advocacy is a small charity providing one to one casework. They offer a free, independent and confidential advocacy service to people aged 18 and over and living in Sefton.

Sefton Advocacy works to ensure that service users are supported appropriately to enable to participate in decisions that affect their lives, giving them the choice and control over their own lives. The team provides help in a range of areas, from general advocacy to specialist services to support people with a variety of needs.
